I hereby give my consent for processing my personal data by the Silesian Opera in Bytom, ul. Moniuszki 21-23, 41-902 Bytom registered in the Register of Cultural Institutions of the Voivodeship of Silesia under no. RIK/O/5/99; the consent refers to processing my personal data indicated in this application form as well as my image and voice, which I submitted in connection with my participation in the 5th International Adam Didur Opera Singers’ Competition for the purposes related to my participation in the Competition and for marketing purposes related to the promotion of the Silesian Opera in Bytom and the promotion of the Competition itself.

Information clause
Due to the fact that you have given your consent for data processing, and pursuant to Art. 13 item 1 and item 2 of the Regulation on the protection of personal data (EU 2016/679) of 27 April 2016, I hereby inform that:
1) the Controller of the data is the Silesian Opera in Bytom, ul. Moniuszki 21-23, acting pursuant to entry in the Register of Cultural Institutions no. RIK/O/5/99.
2) The Silesian Opera has appointed personal data protection officer who might be contacted in:
a) via post: Inspektor Ochrony Danych, Opera Śląska w Bytomiu, ul. Moniuszki 21-23, 41-902 Bytom,
b) via email: iodo@opera-slaska.pl.
3) Personal data of the Participant will be processed on the basis of their consent, for the purposes of organising, promoting and conducting the Competition, and on the basis of the legal provisions related to the Competition’s financial statistical and archiving purposes.
4) The providers of hosting, administration and technical support services for websites and electronic mail will be the recipients of the personal data of the Participant and his/her own pianist. Personal data may be made available to other authorised entities on the basis of legal provisions. The personal data of the Competition’s laureates may be submitted to the the prize funders in order to fulfil the formal requirements for receiving the prizes.
5) The personal data of the Participant and his/her own pianist will be kept for the period necessary due to tax regulations and the provisions of the Copyright and Related Rights Act, as well as for the period required by archiving regulations.
6) The Participant has the right to access and rectify his/her personal data, the right to restrict processing and the right to lodge a complaint on the processing of personal data to the supervisory authority of the President of the Office for Personal Data Protection
7) The Participant has the right to withdraw the consent for the processing of personal data at any time, without affecting the lawfulness of the processing carried out on the basis of consent before its withdrawal.
8) Providing your personal data is voluntary, yet it is necessary for participating in the Competition and for execution of the rights for artistic performances acquired by the Silesian Opera in Bytom.
